Now more than ever, our world needs kindness. Our neighborhoods need care, and many of our community families need help.

To address these challenges our community health workers are helping our community stay healthy. These special team members are like champions for good health. They’re trained to understand what our community needs and provide support where it’s needed most.

They might help people with medication management, teach them how to stay healthy or find solutions to problems that affect health, like not having enough food or a safe place to live.

By working together with these community health workers, we can make sure everyone gets the help they need to be healthy and strong.

We’re working hard to improve healthcare for over 9.4 thousand people in Rice County, Kansas by:

  • Offering guidance, resources, and support for people living with chronic diseases.
  • Connecting community members at risk for type 2 diabetes to a diabetes prevention program and follow up with participation and goals
  • Connecting patients to local, state, and federal resources
  • Offered to any age from prenatal to end of life
  • Help navigate the healthcare system
  • Assistance with housing, transportation, food, utilities, education, and employment.
  • Our services are billed through your insurance, with no out-of-pocket cost to you.
  • Offered throughout Rice County and surrounding county’s
