Creating Healthier Communities

Now more than ever, our world needs kindness. Our neighborhoods need care, and many of our community families need help.

To address these challenges, Rural Health Connections is about helping our neighbors stay healthy—through the people who know the communities best. These special team members are like champions for good health. They’re trained to understand what our community needs and provide support where it’s needed most.

They might help people find doctors or clinics, teach them how to stay healthy or find solutions to problems that affect health, like not having enough food or a safe place to live.

By working together with these community health workers, we can make sure everyone gets the help they need to be healthy and strong.

We’re working hard to improve healthcare for over 9.4 thousand people in Rice County, Kansas by:

  • Improving population health through the increased use of community resources and clinical services for those at high risk for poor health outcomes among populations of focus.
  • Providing diabetes education and goals in hypertension management to those at highest risk for poor health outcomes in populations of focus.
  • Engaging in community outreach and education about COVID-19 and chronic disease screening and health, working in parts of the community where rates of COVID-19 and chronic health conditions are high
  • Connecting community members at risk for type 2 diabetes to a diabetes prevention program and follow up with participation and goals
  • Offering postpartum support to new parents
