  • Verify Insurance Coverage: Contact your insurance provider to understand your benefits and any specific requirements in your plan.
  • Collect Medication Information: Make a list of your medications and dosages to share with your healthcare provider.

Items to Pack in Your Hospital Bag

To ensure you have all your necessary items during your hospital visit, we recommend bringing only essential items such as:

  • Driver’s license or valid photo ID card, a U.S. passport, military ID, or student ID card
  • Insurance and Medicare cards
  • Toiletries
  • Eyeglasses, dentures, and hearing aids
  • Comfortable clothing (loose-fitting is best)
  • A copy of your advance directive if you have one
  • Loose-fitting outfit to wear home

Getting Settled In

Patients will register at the front desk for the following services:

When you initially arrive at the hospital, head to the admissions desk located in the hospital lobby, next to the entrance located on S. Clark Ave.

We’ll ask you to fill out some basic forms, including personal information and medical history. Don’t worry if you’re unsure about anything – we’re here to help.
